Add profile page

Summary: Give user profile page where they can input basic profile info

Requirements:

1. Add profile page entry point to dashboard

2. Within profile page behavior

• User should see several input fields for profile info
• User should be able to click into input fields and add information (free form)
• Provide two main CTAs (Save and cancel)
• When user taps cancel, take user back to dashboard
• When user taps 'save', save the information and store in BE
• User also should have a back option which takes users back to dashboard

3. When user navigates back to profile page at a different time after adding information

• Show profile info that was entered and recorded before
• User should be able to edit fields
• User can click 'save' to save updated info or cancel and 'not save' updated info
• user can click back to go back to profile

4. Fields to fill out within profile page, all in freeform

• Name (First & Last)
• Age
• Emergency contact: show two fields, Name and phone
• Car license plate:
• Medical issues: show larger input box since it will contain more context
