refactor(dashboard): convert commands to generator-based output system

Convert dashboard list, view, and create commands to use the new
async generator pattern with CommandOutput class:

- async func() → async *func() generators
- return { data } → yield new CommandOutput(data)
- Remove json: true from output config (presence of output implies it)
- Regenerate SKILL.md to include dashboard commands
