P1-T12: Improve troubleshooting docs for Zed broker startup timeouts

Description

Add Zed-specific broker timeout recovery guidance to the troubleshooting docs and DocC mirror. The new docs cover the observed 0 tools -> Context server request timeout recovery path, recommend a dedicated broker host for Zed reconnects, and clarify that inactive mcpbridge-broker entries in Xcode Agent Activity are usually historical sessions.
