FU-P12-T1-4: Make IN FLIGHT KPI reflect real in-flight requests in shared-metrics mode

Description

Implement shared-mode IN FLIGHT KPI support by deriving outstanding request count from unresolved request rows in SQLite. Adds test coverage for pending-request behavior and cross-instance aggregation using the same DB file.
