P14: Add 0.4.0 release-readiness tasks and final task gating

Description

Add Phase 14 release-readiness tasks to SPECS/Workplan.md for the four issues found in the 0.4.0 pre-release review, and enforce sequencing so P14-T2 (release metadata/changelog alignment) is the final blocked task after P14-T1, P14-T3, and P14-T4.
